Your margin basis
Off by default the margin is the Drug Tariff price minus the cheapest supplier cost per pack. The clawback and the dispensing fee vary by practice, so turn them on to see the margin on your own basis. You can edit the fee to your practice's own figure. The PSUK APM rebate is a monthly cash rebate on PSUK generic lines only; turn it on to net it off PSUK generic prices, and edit the rate to your own - it varies from practice to practice.

Prescribing wins

Where a different way of writing the same treatment earns your dispensary more on today's cross-supplier market. Value-led guidance within clinically comparable choices, never a substitute for clinical judgement.

Tariff limit

Write generically

high generic Drug Tariff, cheaper source - the difference is yours

Write the generic name on the script. Reimbursement follows the generic Drug Tariff price, so when a brand or supplier costs less to buy, your dispensary keeps the difference.

Ladder detail: +£/script = today's generic Drug Tariff minus the cheapest source's nett cost today (net of manufacturer (MDS) scheme rebates). A source marked (guide price) has no live offer today. Check pack size and licence before switching.
Switch only where clinically appropriate and re-counsel as needed.

Within a pharmacological class these drops are broadly comparable, but preservative content, instillation frequency and established response make a glaucoma switch an individual clinical decision, usually with the patient’s ophthalmologist, never an automatic substitution. A patient on a preservative-free unit-dose product is usually on it for a reason, so it is not interchangeable with a preserved bottle. (NICE NG81: a prostaglandin analogue is first-line for open-angle glaucoma.)

Options are grouped ONLY within a potency band, so a change here keeps the same potency; it is never a step up or down in strength. Creams and ointments appear in the same band because the base is part of the same prescribing decision as the potency and the site, and every row states which it is - an ointment is greasier and more occlusive, better on dry lichenified skin and less well accepted on weeping, flexural or facial areas, so read the vehicle as well as the figure. Pack size and the treated site still matter, and the packs in a band are not all the same size. A switch should be counselled. (BNF potency table: clobetasone butyrate 0.05% and betamethasone valerate 0.025% are classed MODERATE.)

Options are ranked only within an identical composition - a different progestogen or a different strength is a prescribing decision, never a dispensing swap, and no ladder here crosses one. Phasic and monophasic preparations are separate dm+d products and never appear in the same ladder. (FSRH CEU Combined Hormonal Contraception; BNF; dm+d virtual product composition)

Transdermal HRT has been subject to repeated national supply problems, so the honest question is usually WHICH OF THESE CAN YOU ACTUALLY GET rather than which earns most - this ladder leads on what is in stock today for that reason. Patches, gels and spray are not dose-equivalent to each other and a change between them is a prescribing decision. Starch-based and gum-based thickeners are NOT interchangeable: they behave differently in the mouth, reach a stated IDDSI level differently, and a change needs the speech and language therapist's thickening instruction re-written. Only the starch powders are ranked here, and only because dm+d gives them one shared composition; every gum-based product is deliberately absent rather than silently mixed in. Dry-eye lubricant drops are largely interchangeable symptomatic treatments and standardising suitable patients onto the best-earning option is a reasonable formulary choice - each patient's tolerance and severity should still guide the decision. Every option here is the same molecule at the same strength, so these are brands of one product rather than different pills. Desogestrel is not interchangeable with a levonorgestrel or norethisterone progestogen-only pill, and no ladder here crosses one. Some desogestrel brands are also sold over the counter under a pharmacy licence; the reimbursement shown is the prescription pack we price. (FSRH CEU Progestogen-only Pills; BNF; dm+d virtual product composition)
Switch only where clinically appropriate and re-counsel as needed.

Fewer tablets

same total daily dose, fewer and larger tablets - the Drug Tariff difference is yours

Some medicines pay more per milligram at the larger strength. Where that is true, writing one bigger tablet instead of two smaller ones keeps the patient on exactly the same daily dose, gives them fewer tablets to swallow, and leaves your dispensary better off. Every row below compares 28 days on each option, spells out the arithmetic when you open it, and only appears while both strengths are priced and in stock today. Value-led guidance within clinically comparable choices, never a substitute for clinical judgement.

PA items

Injections, vaccines and self-administered injectables you already administer or dispense, and the claiming plays on them. Switch between PA items (personally administered in the practice, reimbursed at the Drug Tariff basic price plus the dispensing fee) and Double VAT (self-administered injectables the patient takes away, where the VAT is recovered twice). These are claiming plays on lines you already administer or dispense, not prescribing switches.
Checking you are recovering this VAT. The NHS pays the PA VAT allowance as a VAT credit line under Drugs (Dispensing) on your monthly FP34 / GPP drug statement. If that line is missing, or looks low against the volume of PA items you administer, you may be under-claiming, so it is worth checking with whoever prepares your PCSE submission.
Want your exact figures? Send us your own ePACT2 or PD4 export and we will work them through with you: [email protected].
The average discount deduction (11.18%) is an FP34 practice average and varies by practice; vaccines and other zero-discount lines will differ. These figures follow your view settings: the clawback and the dispensing fee apply only when you switch them on in the YOUR VIEW bar above, where the fee (default £2.04, the current dispensing-practice fee scale - practices that do not dispense are paid on a different scale) is editable. Price concession reports

These generic lines cost more to buy than the NHS currently reimburses through the Drug Tariff, and no price concession is yet in force. Reporting them to Community Pharmacy England (CPE) is how dispensers flag which lines are loss-making: CPE collates these submissions to evidence where the Drug Tariff is set too low, secure a price concession for the month, and push for appropriate reimbursement. The more practices report a line, the stronger the case. Tick the ones you actually bought, confirm the price and pack, then generate your report for CPE.
RxMargin Live Market. Universal dispensary margin intelligence built from wholesaler pricing, the Drug Tariff and monthly price concessions, refreshed from the live capture every hour through the working day. Buy prices are net of the national MDS scheme rebate for each drug and wholesaler. Margin per pack is the Drug Tariff reimbursement price minus the cheapest in-stock supplier cost per pack; figures exclude practice-specific loyalty bonuses and spend-tier discounts, which are individual to each practice. Indicative figures to support buying and prescribing decisions; not financial advice and not a guarantee for any individual practice. Built 11/08/2026.
